Partnership in Action for Trinity Health (PATH) is a local group of committed community members and organizations working together in Trinity County, California to improve the health of all Trinity County residents.  The PATH coalition was created in 2017 to foster collaboration between members of the community and organizations that want to build a healthier and safer Trinity County.

The mission of PATH is to promote a healthy, safe, and active Trinity County through partnerships, innovation, and action.

PATH is facilitated by the Human Response Network Tobacco Free Trinity Program.
